Ether the makers for the game forgot to make the graphics for that monster or the game lag when you were entering the area and skipped the graphic image for that 1 guy.
Na its happened to me a few times, the game accidentaly deletes that image. If it happens the next time I go into the game it takes a while to fix everything and find whats missing
Still was pretty funny the first time I saw it :D
reminds me of the preview event, when /preorder got you the spear, but it didnt have the graphic. the picture was of a man with a construction hat on, and the equipped immage was a white cube. thus: fridge cannon XD
